Community Health Navigator & Social Prescribing Specialist

NHS Jobs is seeking a Health and Wellbeing Link Worker to support patients in the community, promoting prevention and a holistic approach to health. You will work with GP practices and partner organisations to connect people with local groups, services and social prescribing options.

You will travel between practice sites, attend meetings, and participate in mandatory training to ensure high standards of care and information governance.
