Job Description:
East London NHS Foundation Trust is seeking a dedicated professional to work with individuals and their families, providing information, signposting and onward referrals to support the person living with dementia. You will work closely with carers, referrals to the carer’s centre and other services, and contribute to personalised care plans reviewed before discharge to the GP.
You will support the Bengali Cognitive Stimulation Group and GRACE interventions within the MDT.
