Care Navigator: Prison-to-Community Health Coach

Rethink Mental Illness invites applications for a Care Navigator to support individuals leaving prison and transitioning to community care. The role focuses on connecting people with health services, improving wellbeing, and reducing health inequalities through coaching-based interventions.

You will work in a prison setting at HMP Millsike, with a 35-hour week, 0800-1600, Monday to Friday. The post requires NVQ3 (or equivalent) and experience in mental health and social care.
