Community Healthcare Support Worker: Crisis & Home Hospice

Oxford University Hospitals NHS Foundation Trust is recruiting a Senior Health Care Support Worker in the City/ South Oxfordshire area. You will join the Crisis Assessment and Response Team and Home Hospice Care Team to support patients at home, help prevent hospital admissions, and respond to urgent referrals on a 7-day rota.
