Primary Care Engagement Officer

Thrive Tribe is recruiting a Primary Care Support Officer to support Stop Smoking services across County Durham. You will engage with Primary Care partners and develop referral pathways into the service.

You will build strong links with primary and secondary care, promote the service, arrange practice visits and newsletters, and report on engagement outcomes to leadership. This role offers hybrid working from our County Durham base and a collaborative culture.
