Leaders In Care Ltd is seeking a Registered Nurse to take on the Nurse Case Manager role supporting autistic adults across Manchester’s supported living and community services. The role offers flexible part-time hours (15 hours over two days) with hybrid working and a Manchester-based caseload.
You will conduct clinical assessments, develop care plans, provide specialist input, ensure safeguarding and risk management, and collaborate with families and multidisciplinary teams.
