Recovery & Care Navigator – Community Health

Change Grow Live in Norwich is recruiting a Recovery Coordinator to support individuals through treatment and recovery journeys. The role involves group work, assessments, and coordinating care across services, with regular travel and a 37.5 hour week.

The ideal candidate will understand substance misuse, have strong communication skills, and be capable of working independently within a collaborative, supportive team. A driving licence is essential.
