About the Role
As a Social Care Facilitator within the Mental Health Team, you will undertake Care Act assessments, support planning and reviews for adults with mental health needs. You will work with individuals, families, carer’s and partner agencies to promote wellbeing, maximise independence and ensure access to appropriate services and community resources.
You will use a strengths-based approach to identify what matters most to individuals, helping them build upon their existing capabilities, support networks and community assets while ensuring statutory duties are fulfilled.
This role offers the opportunity to work with a diverse range of adults, including individuals experiencing severe and enduring mental illness, complex emotional needs, co-existing health and social care challenges, and those requiring support following discharge from hospital or secondary mental health services including those who are eligible for s117 aftercare under the Mental Health Act.
Key Responsibilities
- Undertake Care Act 2014 assessments (Section 9) to identify eligible care and support needs, strengths, personal outcomes and opportunities to promote independence.
- Complete Carer’s Assessments (Section 10 Care Act 2014), recognising and supporting the needs of unpaid carers.
- Undertake Section 117 Aftercare assessments and reviews, working collaboratively with health partners to identify aftercare needs arising from an individual’s mental disorder and to support recovery following detention under the Mental Health Act.
- Participate in the planning, review and coordination of Section 117 Aftercare arrangements, ensuring services remain appropriate, effective and focused on reducing the risk of deterioration and readmission.
- Develop, implement and review person-centred support plans and Section 117 aftercare plans, promoting recovery, wellbeing, social inclusion and independence.
- Work closely with Community Mental Health Teams, NHS Trust colleagues, Social Workers, Approved Mental Health Professionals (AMHPs), housing providers, advocacy services and voluntary sector organisations to deliver coordinated support.
- Support adults to access community resources, employment, education, meaningful occupation, housing support and preventative services that contribute to positive mental health outcomes.
- Contribute to multi-disciplinary assessments and care planning meetings, ensuring the social care perspective is represented, and individuals’ voices are heard.
- Support effective hospital discharge planning and transitions between inpatient, community and social care services.
- Identify and respond appropriately to safeguarding concerns, undertaking risk assessments and escalating concerns in accordance with safeguarding procedures.
- Apply relevant legislation including the Care Act 2014, Mental Health Act 1983, Mental Capacity Act 2005, Human Rights Act 1998 and associated statutory guidance in day-to-day practice.
- Maintain accurate, timely and legally compliant case records, assessments, support plans and review documentation.
- Manage an allocated caseload effectively, prioritising work according to complexity, risk, statutory timescales and service demand.
- Promote a strengths-based, recovery-focused and trauma-informed approach, empowering individuals to build resilience, maximise independence and achieve outcomes that matter to them.
