Care Coordinator — Lead Field-Based Home Care

Care By Us is seeking a Care Coordinator to organise and deliver high-quality in-home care within the community. You will manage care workers, assess care plans, and support field staff while maintaining strong relationships with clients and teams.

The role requires a valid driver’s license and access to your own vehicle due to travel, with a Monday to Friday 9am–5pm schedule and additional on-call duties on a rota basis. An Equal Opportunities employer, we value community care excellence.
