Job Description:
Wokingham Borough Council is seeking a Social Care Practitioner to join the Integrated Mental Health Team in Adult Social Care on a full-time, permanent basis. You’ll work alongside experienced social care professionals, supporting adults with serious or suspected mental health conditions and their carers.
Under supervision, you’ll assess needs, develop support plans, spot safeguarding concerns, and help people stay safe, independent and connected to their community.
