Nurse-Led Clinical Package Lead – Home Care

Brightening Lives in Whitby is seeking an experienced Clinical Package Lead to coordinate a home-based care package for a lady with an acquired brain injury. You will lead the team, ensure safe care and maintain the individual’s independence and dignity.

You will work closely with the Clinical Lead Nurse, Personal Assistants and wider healthcare professionals to keep care plans current, monitor delivery, and promote a positive team culture within Brightening Lives.
