Senior Care Home ACP: Clinical Lead & Quality Champion

Blackpool Teaching Hospitals NHS Foundation Trust is recruiting an Care Home Advanced Clinical Practitioner (ACP) to provide clinical leadership for the PCN Care Home Team and to oversee day-to-day operations within the service.

You will lead multidisciplinary reviews for high-risk residents, develop personalised care plans, and support advance care planning while promoting quality and safety across the care home network.
