Senior Community Matron: Proactive Care & Care Home Support

Northumbria Healthcare NHS Foundation Trust is seeking a Community Matron to join our Community Nursing Teams across South East Central Northumberland. The post involves advanced clinical assessment, proactive case management for highly complex patients, and coordinating care with Specialist Nurses, Medical colleagues, Palliative Care, Intermediate Care and Social Care to reduce hospital admissions.
