RT Education is recruiting an experienced Hospital Team Manager for our Adult Social Care team in West London. You will lead a multidisciplinary team in a hospital setting and oversee discharge planning to ensure safe and timely transitions for adults with varying needs.
You will manage complex cases, support social workers with assessments and care planning, and ensure compliance with safeguarding and statutory requirements. Hybrid working with three days in the office.
