Paediatric PATCH Nurse – Home Visit Specialist

Chelsea and Westminster Hospital NHS Foundation Trust is recruiting a PATCH Nurse to join the Paediatric Hospital at Home team. You will assess and manage acutely unwell children at home, using face to face, virtual and telephone consultations with escalation to hospital teams when needed.

You will drive to patient visits, hold a licence for lone working and work within the Trust’s high standards of care. The role supports families and requires commitment to equity and safe, compassionate care.
