Community Mental Health Nurse: Complex Caseload & Mentorship

CNWL NHS Foundation Trust in Milton Keynes is seeking a Community Mental Health Nurse to join a multidisciplinary team. The post holder will assess, develop and evaluate care plans using Dialog+ and act as keyworker for a complex caseload.
