Job Description:
Birmingham Community Healthcare NHS Foundation Trust seeks an experienced Practice Pharmacist to support safe and cost‑effective use of medicines within the practice. Responsibilities include repeat prescribing, medicines reconciliation after discharge, and medicine reviews to optimise treatment and adherence.
You will respond to prescription queries, monitor high‑risk medicines, advise patients and carers, and work with GPs and hospital teams to ensure continuity of care.
