Community Therapy Assistant Practitioner, Home Care

Norfolk Community Health and Care NHS Trust in Norwich is seeking an experienced Therapy Assistant Practitioner to join the Central Norfolk Home First Hub. The role involves triaging therapy referrals and supporting discharge to assess pathways across a 7-day operation, including home and care home visits.

You will work within an integrated multidisciplinary team and rotate across community settings, working closely with OT, Physio, SW colleagues, and Integrated Care Coordinators.
