Discharge Social Worker – Hybrid, Fast-Paced Role

Leaders in Care is recruiting a Community Care Social Worker to join a supportive Pathway 3 service in Bangor. You’ll help discharge hospital patients safely by coordinating plans with families, health colleagues and community providers.

You’ll manage a fast-paced caseload, complete strengths-based assessments, and ensure appropriate community support is in place. This is a hybrid role with office days and field visits, backed by a specialist agency and strong payroll support.
