Transition to Adulthood Social Worker – Bracknell

Eden Brown Synergy is currently recruiting an experienced Social Worker to join the Approaching Adulthood Team in Bracknell. The role focuses on Care Act assessments for young people transitioning to adulthood and developing person-centred support plans to promote independence.

You will work with families, schools, health and social care colleagues across agencies, ensuring timely transition planning and clear pathways into adult services.
