Neurology Specialist Nurse: Triage, Education & Care Leader

Norfolk Community Health and Care NHS Trust is seeking a nurse specialist to triage referrals for people with long-term neurological conditions, coordinating care at local, regional and national levels.

You will assess, monitor, educate and support patients, evaluate findings, and develop goal plans; you will monitor drug therapies and advise on changes in line with professional guidelines.
