Band 6 District Nurse – Integrated Community Care Lead

Croydon Health Services NHS Trust is recruiting a Band 6 District Nurse/ Caseload Holder to work in our Integrated Care Teams across Croydon. You will provide patient-centred nursing care in patients’ homes, drawing on specialist district nursing skills and leadership to coordinate care and support for complex cases.

The role requires a Registered Nurse with a District Nursing qualification or relevant community nursing experience, and a valid UK driving licence with access to a car for work.
