West Hertfordshire Teaching Hospitals NHS Trust is recruiting for a Band 8b Clinical Programme Lead to spearhead the Care Home Dementia Support Service across South and West Hertfordshire. The role focuses on mobilisation, strategic development and inter-agency collaboration to improve dementia care in care homes.
You will work closely with the Alzheimer’s Society and partners to deliver person-centred care, reduce avoidable hospital admissions and drive quality governance.
