Community Nurse – Home Care with Impact (Band 5)

HCRG Care Group is seeking a Community Nurse (Band 5) to deliver high-quality, person-centred nursing in patients’ homes from Amesbury Health Centre. You will work in line with national standards and collaborate with GPs and MDT members to assess, plan and deliver care, promoting independence and positive health outcomes.

You will require an NMC registration, be able to work autonomously within a multidisciplinary team, and have a full UK driving licence with car access.
