Rapid-Access Mental Health Practitioner

East London NHS Foundation Trust is seeking an experienced Band 6 Mental Health Practitioner to join a front‑door service. You will assess referrals from GPs and acute services and help people access timely support in the least restrictive environment possible.

You will work directly with service users, carers and families to assess risk, formulate care plans and connect with community resources that promote recovery, resilience and independence in a fast-paced, collaborative setting.
