Job Description:
Royal Bournemouth and Christchurch Hospitals NHS Foundation Trust is seeking a Multi Professional Lead for Discharge to join the UHD Integrated Discharge Team for a 12 month fixed term contract or secondment.
The post holder will lead discharge practice for complex cases, act as an autonomous discharge expert within the ward team, and promote Home First and Discharge to Assess principles to achieve safe, timely patient discharges.
