Outreach Mental Health & Wellbeing Practitioner – Flexible 35hrs

Change Mental Health is seeking a Mental Health and Wellbeing Practitioner (Outreach) to join our Central services team in Edinburgh. This community-based role supports adults affected by mental illness to build confidence, resilience and greater independence, primarily through one-to-one work in clients’ homes and communities.

You will connect people with local opportunities and services, collaborate with partners, and help identify and respond to safeguarding or wellbeing concerns.
