Primary Care Nurse Practitioner or Physician Assistant

Role: Provide patient-centered primary care as a Nurse Practitioner or Physician Assistant within a collaborative, physician-led team.

Key Responsibilities

• Conduct comprehensive patient assessments and develop individualized care plans.
• Diagnose and manage acute and chronic conditions.
• Order, interpret, and act on diagnostic tests.
• Prescribe medications and provide appropriate treatment interventions.
• Coordinate care across specialties to ensure continuity and seamless transitions.
• Deliver preventive care, health education, and counseling for individuals and families.
• Collaborate with physicians, behavioral health, and ancillary services to support integrated care.

Required Qualifications & Skills

• NP or PA licensure/credentialing to practice in primary care.
• Strong clinical assessment, diagnostic reasoning, and treatment planning skills.
• Effective communication and relationship-building with patients and families.
• Ability to collaborate within a multidisciplinary healthcare team.
