Early Years Transition & Inclusion Specialist

Cardiff Council is seeking a Transition Worker within the Disability Inclusion and Early Years team. The post focuses on working with parents/carers to ensure children with emerging and/or additional needs access their entitlement and make good progress in transitions to childcare and school.

The role requires strong organisation, motivation and partnership working with families, schools and health/education professionals.
