An exciting opportunity has arisen to join the Children’s Continuing Care Team at Great Western Hospitals NHS Foundation Trust, supporting children and young people with complex medical needs to live safely and happily within their home and school environments.
As a valued member of our dedicated and compassionate team, you will provide direct clinical care and support tailored to the individual needs of the children and families we serve. Your role may involve providing overnight care in a child’s home, enabling families to rest and recharge, or supporting a child within their educational setting, helping them access learning and reach their full potential.
We are looking for caring, enthusiastic and reliable individuals who are passionate about delivering high-quality care. Following a comprehensive competency-based training programme, you will be confident working independently as a lone practitioner within the community, while remaining fully supported by our experienced and friendly team.
Please note: Part-time or term-time hours do not meet the required salary threshold for sponsorship and therefore would not be eligible for sponsorship

Detailed job description and main responsibilities
To provide skilled high-quality care to children and young people with complex medical needs, in the home or community setting.
To carry out personal care and delegated clinical tasks, providing support for children with complex health needs and those dependent on technology such as ventilators, feeding tubes and oxygen.
To follow individualised care plans developed by the nursing team, having gained competency through an intensive and on-going training programme and with support and guidance from registered nurses.

Person specification
Qualifications
- One of the following: NVQ 3 in Health and Social Care, Childcare or equivalent.
- NNEB
- Experience equivalent to the above qualifications
- Level 2 Functional Skills in English and Math’s or equivalent
- Completion of Level 3 Diploma in Healthcare Support (that maps to the Apprenticeship Standard for Senior Health Care Support Worker), or
- Equivalent experience together with Level 2 qualification in childcare, Health and Social Care or equivalent and working towards Level 3
Experience
- Demonstrate the ability to work without direct supervision once trained and deemed competent to provide delegated clinical skills.
- Experience of working with children and young people with special needs
- Experience of team working
- Experience of working variable shift patterns to include nightshifts

Payment for DBS checks for bank posts must be settled prior to commencement. Apprentices are exempt from DBS payment.
