Autonomous Frailty Nurse Specialist – Community Care

NHS trust in East and West Sussex seeks an experienced Specialist Community Frailty Nurse (Band 7) for a 12-month fixed-term post. You will autonomously manage a personal caseload, provide expert clinical care for frailty patients, and work with GPs, community nurses, and geriatricians to prevent hospital admissions.

Must have RN qualification, advanced prescribing or willingness to train, and a strong background in community-based frailty care.
