Cardiac Rehab Lead Practitioner – Flexible Hours

Bristol NHS Foundation Trust is seeking an experienced cardiac rehabilitation enhanced practitioner (Band 6) to join the MDT across hospital sites and community venues in Bristol and Weston. You will provide autonomous clinical assessments and lead an eight-session outpatient programme, coordinating care with GP’s, psychologists, dieticians and pharmacists.
