Lead Nurse, Hospital at Home & Community Care

Bedfordshire Hospitals NHS Foundation Trust is seeking a Hospital at Home Team Nurse to work with the H@H teams, delivering skilled nursing for patients suitable for discharge and continuing treatment at home.

The role includes education for patients and carers, multidisciplinary collaboration, and developing pathways to enable safe care outside the acute hospital setting. Travel between hospital sites may be required.
