Social Worker – Adults Integrated Reviewing – Leeds

We are seeking a dedicated and compassionate Social Worker to join our Adults Integrated Reviewing team in Leeds. This role is pivotal in ensuring that our adult service users receive the highest quality of care and support tailored to their individual needs. As a Social Worker, you will conduct comprehensive reviews of care plans, collaborate with multidisciplinary teams, and advocate for the rights and well-being of vulnerable adults in our community.

Key responsibilities and duties include:

• Conducting thorough assessments and reviews of adult service users’ care needs.
• Developing and implementing person-centered care plans in collaboration with service users and their families.
• Working closely with healthcare professionals, community organizations, and other stakeholders to ensure holistic support.
• Monitoring and evaluating the effectiveness of care plans and making necessary adjustments.
• Providing advice, guidance, and support to service users and their families regarding available services and resources.

To be considered for this role, candidates must possess the following essential qualifications and skills:

• A recognized Social Work qualification (e.g., Degree in Social Work or equivalent).
• Current registration with Social Work England.
• Experience in adult social care, particularly within integrated reviewing or assessment roles.
• Strong communication and interpersonal skills, with the ability to build rapport with diverse individuals.
• Excellent organizational skills and the ability to manage a varied caseload effectively.
