Senior Clinical Practitioner

Overview

In this Band 7 role at Medway Maritime Hospital, you will deliver hospital-based mental health care within a busy, multidisciplinary setting. You’ll handle urgent and routine referrals, complex assessments, and risk management to support patients with diverse needs. A key focus is teaching and supporting acute hospital staff to improve mental health understanding and holistic care. You’ll contribute to service targets, quality standards, and ongoing improvement through audits and IT-enabled documentation. This position offers a meaningful opportunity to shape liaison psychiatry practice and patient experience.

Responsibilities

• Assess and respond to acute hospital referrals (CORE24: 1-hour triage, 4-hour full assessment)
• Conduct biopsychosocial assessments for various mental health conditions
• Manage complex caseloads with co-morbid physical and mental illness
• Lead risk assessment and safeguarding processes
• Coordinate care plans where social needs outweigh other needs
• Provide specialist assessment and intervention for adults (18+) with co-morbid conditions
• Deliver education, training, and advice to acute hospital staff and colleagues
• Supervise Band 6 practitioners and support their development
• Lead change for LPS accreditation and CORE24 service delivery
• Develop clinical audits and lead service improvements (PREM/CREM)
• Conduct inductions for new clinicians
• Build strong cross-team relationships within Kent and Medway Partnership Trust
• Support service integration across hospital and community settings
• Drive involvement in clinical areas such as neurodiversity, substance misuse, dementia/delirium, self-harm
• Lead in specific clinical area and contribute to service-wide improvements

Key requirements

• Senior nurse (band 6 or above) with at least 1 year of experience
• Experience identifying, assessing and managing clinical risk
• Current UK professional registration (RMN, OT, or relevant clinical qualification)
• Post-registration therapy qualification or willingness to work toward one
• Ability to develop and deliver training
• Strong therapeutic alliance skills and engagement in treatment
• Computer literate and able to work under own initiative
• Effective liaison with colleagues and multidisciplinary teams
• Disclosure to DBS as required
