The Support Worker (SW) provides safe, compassionate and person-centred support to children, young people and adults in their homes and within the community. Responsibilities include supporting daily living, community activities, independence and wellbeing, following individual support plans, maintaining accurate records, safeguarding service users, and reporting any concerns or changes in needs to the appropriate manager.
