Advanced Clinical Practitioner

Overview

As an Advanced Clinical Practitioner in the Virtual Ward and Hospital at Home Service you will lead advanced assessment, prescribing and treatment to support acute care at home. You will collaborate with cross-site teams to reduce hospital stays and enable early discharge, expanding innovative care pathways via remote technology. The role involves MDT participation, prioritising deteriorating patients, and contributing to audit and service development. Responsibilities

• Lead advanced practice assessment, diagnosis and treatment planning for virtual ward patients
• Prescribe and manage medications, including routine drug chart preparation for discharge
• Review newly admitted patients and participate in MDT sessions and specialist reviews
• Identify deterioration, escalate appropriately and document clinical decisions
• Conduct audits and research to improve practice and support new knowledge
• Support initial assessment and management of patients in high acute beds and in home settings
• Adopt a flexible approach to stepping up/stepping down patients across virtual ward, urgent care and A&E

Key requirements

• Health Assessment Qualification at Level 7
• Experienced V300 Non-medical prescriber
• Masters in Advance Clinical Practice
• Full UK driving license
• Access to own vehicle for work purposes
• Current UK professional registration
