Mindworks Hub Youth Justice Clinical Practitioner

Overview

In this role you will manage caseloads of vulnerable young people aged 10–18 and work to improve mental health and wellbeing within a community setting. You will collaborate with youth justice, housing, education and other services to deliver goal-based assessments and interventions. The role includes outreach and signposting to appropriate supports, with a focus on safeguarding and joint decision-making with families. You will work across Surrey & Borders, contributing to high-quality, locally delivered care close to home. Responsibilities

• Respond to consultation requests and assess young people to provide goal-based mental health interventions
• Collaborate with housing, education, substance misuse services and other supports for young people
• Safeguard and promote welfare of young people; follow SABP safety and lone-work policies
• Engage in annual performance review and personal development planning
• Conduct outreach to meet young people in their chosen environments
• Liaise with families and other professionals; attend case conferences and planning meetings
• Signpost to appropriate services and support re-engagement with school/college
• Carry out risk assessments and document notes and risks accurately in the system
• Demonstrate flexibility for project work, rotations, and absence cover
• Possess and use a driving license for travel to sites across the county

Key requirements

• Degree or diploma in Mental Health Nursing
• Current NMC registration
• 2-year post-qualification experience working with young people in the community
• Experience with mental health assessments, risk assessments, and risk management planning
• UK driving licence and car for work
