Community PD Clinical Nurse Specialist – Home Care Lead

King’s College Hospital NHS Foundation Trust is seeking a Band 6 Community Peritoneal Dialysis Clinical Nurse Specialist to join our PD Team. You will deliver expert clinical care and education in patients’ homes and community settings, supporting independence while providing life-sustaining renal replacement therapy.

Working with a multidisciplinary team, you will lead PD care, advise on guidelines, maintain communication with primary care, and be an active member of the team.
