Transitions Social Worker: Empowering Young Adults

St Helena Public Service invites applications for a lead role in supporting children and young people as they transition to adulthood. You will manage a caseload across Children’s and Adult Social Care, coordinate assessments and transition plans, and work with families to ensure positive long-term outcomes.

You will be required to undertake high-quality assessments, collaborate with partner services, and participate in multi-agency meetings to secure a smooth progression to adulthood.
