What you will do:
- Promote independence and wellbeing for adults and their carers
- Safeguard individuals at risk and respond to safeguarding concerns
- Assess needs in line with the Care Act 2014
- Develop and deliver person-centred care and support plans
- Work collaboratively with service users, carers, and partner agencies
- Monitor and review care to ensure high-quality, effective support
- Contribute to service development and continuous improvement
