Overview
In this role you support the Virtual Wards and Multidisciplinary Team to deliver proactive community health care coordination. You will handle a wide range of administrative tasks, support MDT processes, and enable effective patient care through data management and liaison with health and social care services. The position offers development through a structured training package and cross-team collaboration within a values‑driven, social enterprise environment. You will work with PCNs to improve joint working and patient outcomes.

Responsibilities
- Receive referrals from PCN teams and coordinate appropriate responses
- Liaise with statutory and voluntary services to arrange patient support
- Plan and organise activities (staff rotas, events, meetings)
- Assist patients with appointments and incidental contact
- Manage equipment, servicing, and records; handle petty cash and procurement
- Contribute to data reporting and audits using structured systems
- Record interventions in electronic systems and support report generation
- Provide induction and guidance to new starters; supervise and develop staff as needed
- Make triage judgments to route referrals to the right service at the right time
- Support marketing and cross-team collaboration within the Integrated Care System
Key requirements
- Experience of office procedures within an administration role
- Experience handling confidential information
- Proven administrative experience
- Excellent IT skills (Microsoft Office, Excel)
- Ability to prioritise, organise and meet deadlines
- Strong communication and listening skills
- Experience with data entry and data management
- Ability to work independently and in a team with some supervisory duties
- Desirable: experience in Multidisciplinary teams; recruitment, selection, and HR management procedures
