In this Bank ACP role, you deliver high-quality, individualized care for adults in flexible sessions across urgent treatment and primary care settings. You work independently while collaborating with the Primary care Multidisciplinary Team to meet patient needs. The post includes occasional home visiting and a holistic care approach, improving access and continuity of care. Responsibilities

• Provide high-quality, individualized care to patients and families
• Deliver safe, effective care across flexible sessions (Mon–Sat)
• Collaborate with the Primary care Multidisciplinary Team
• Conduct home visiting consultations as required
• Operate within a holistic care model to meet treatment needs

Key requirements

• Advanced Care Practitioner Qualification or equivalent
• Non Medical Prescribing Qualification
• IR(ME)R course
• Evidence of continuing professional development
• Current UK professional registration
• Experience of seeing patients in a sole environment, minor illness and treatment room conditions

• Excellent interpersonal and communication skills
• Reliable and flexible attitude
• Ability to engage with stakeholders across the organisation
• Excellent IT Skills
• Good working knowledge of Emis
• Car Driver
