Overview

You’ll assess and review care and support needs to deliver high-quality, person-centred services that promote independence and wellbeing. You’ll conduct Care Act assessments, arrange and review care packages, and support decision-making under the Mental Capacity Act 2005. Working with health professionals and partners, you’ll do telephone assessments, home visits, and participate in multi-disciplinary meetings to ensure effective services. This role sits in the Learning Disability and Autism Service, contributing to transformation and better outcomes. You’ll be empowered to share ideas and develop your practice while helping people live more independently.

Responsibilities

  • Conduct Care Act assessments and review care packages
  • Arrange and review Continuing Healthcare (CHC) checklists
  • Support decision-making in line with the Mental Capacity Act 2005
  • Carry out telephone assessments and home visits
  • Attend multi-disciplinary meetings with health professionals and partner organisations
  • Contribute to service development and improvement initiatives
