HIOW Healthcare is seeking an inspirational community nurse to deliver high-quality care in patients’ homes across the city. Nurses work autonomously and within a supportive team, ensuring patients receive the best possible care at home and help prevent hospital admissions.
The role involves managing a caseload of adults with long-term conditions, complex wounds, palliative needs, and discharge facilitation. Shifts include 08:00–16:00 and 14:00–20:00, with 1 in 3 weekends and bank holidays.
