The Bordon and Petersfield Community Older Persons Mental Health (OPMH) Team operates from the main team base at Chase Hospital and covers Petersfield, Bordon and some of Waterlooville working 9-5 Monday — Friday. The team is approachable and open. We support each other and have developed a culture of honesty and integrity.
Please note this role is part time, 22.5 hours a week.
This is a lively, dynamic and busy multi-disciplinary team which provides care coordination and recovery focused care for individuals with both organic/functional mental health needs. This is an opportunity to demonstrate your skills as a Healthcare Support Worker, who is focused on continually improving outcomes for patients.
You will play an important role in delivering excellent care for our patients and working as part of the team to provide the right support and assistance to our patients in their recovery. You would be joining a supportive team that work together closely.
Due to the nature of this post, you will need to carry out home visits and attend patient meetings therefore, a car driver is essential with access to own car.
