Adulthood Transition Social Care Navigator

Somerset Council is seeking a candidate to join the Transition to Adult Social Care team, assisting young people and families as they move from Children’s to Adult Social Care. You will assess care needs, plan with families, and coordinate access to community and health services, with strong emphasis on rights and independence.

You will build trust, communicate clearly, and work alongside Social Workers, health professionals and education colleagues.
