Rochford Primary Care Network is seeking a part-time Healthcare Assistant to join the Population Health team and support NHS Health Checks and related community care.
Under the supervision of the Population Health Practitioner, you will perform baseline observations, BMI, blood pressure, and health promotion activities. You will assist with chronic disease management in a community setting, including outreach and signposting to local services.
