Onsite – Clinical Pharmacist IP – Carlisle

Overview

In this role you support day-to-day operations within a general practice as an onsite clinical pharmacist, delivering expert medication care and guidance while collaborating with a multidisciplinary team to improve patient outcomes. You help resolve medication queries, lead medicines optimisation and contribute to service improvement. This is a patient-centred, practice-focused opportunity within a growing pharmacist-led network that values collaboration and excellence.

Responsibilities

• Resolve medication-related queries
• Polypharmacy reviews and SMRs
• Face-to-face medication reviews
• Utilise skill mix within the team
• Improve patient outcomes and access to healthcare
• Support staff with prescription and medication queries
• Medicine reconciliation and post-discharge review
• Conduct medical audits and drive service improvement
• Provide expert advice on medications to clinicians and patients
• Deliver clinical leadership on medicines optimisation and quality improvement
• Ensure appropriate and efficient use of medication
• Educate patients on medication and disease prevention
• Understand QOF, CQC and local incentive schemes
• Create and follow the clinical pharmacy programme per policy

Key requirements

• Minimum 12 months experience in general practice/primary care
• GPHC registration (relevant degree/qualification)
• Independent prescribing qualification
• Experience with EMIS or System1
• Completed CPPE Primary Care pathway or nearing completion
• Able to commute to onsite location
