Discharge & Care Social Worker (Adults) – Fixed Term

Wirral Council is recruiting a Social Worker for a fixed-term maternity cover, split between Clatterbridge Hospital CICC and Arrowe Park Hospital TOCH. You will support safe discharges, provide person-centred social care, and contribute to care planning for adults and carers.

You will work in a dynamic MDT, uphold Care Act 2014 principles, and mentor newer staff while managing complex situations and maintaining high-quality practice.
